Fiendish Amethyst is a trinket in Fiend Folio, a mod for The Binding of Isaac.

Upon entering an uncleared room, 4 amethyst Fiend minions are spawned that deal 20% of Isaac's damage per tick to enemies, and disappear after 10 seconds.
