After paying the toll, the Ferryman will transfer Isaac to a new location called Fragment.

Inside Fragment, Isaac will see a Death statue and 4 portals that lead to different unique rooms where Isaac needs to protect (and sometimes find and carry) a Lost Soul, clear a room, and exit it. Upon clearing a room it will pay out with a reward as usual.

Upon carrying the Lost Soul to the main room of Fragment, it will reward Isaac for saving and navigating it to the Ferryman.

Rewards include regular pickups and rarely trinkets or consumables, always in quantity of 3.

After successfully doing this all 4 times without losing a Lost Soul, the Death statue will disappear and reward Isaac with a random item from the Death Deal item pool.

At any time Isaac can leave Fragment by jumping back to Ferryman's boat and returning to the room the Ferryman transferred him from.

Ferrymen always appear on all regular floors up to Cathedral/Sheol in dead-end rooms, usually having some pickups, other beggars, or even rarely a purchasable item from the Treasure Room item pool along with some shopkeepers.
