The Blessing of The Fool of Worms is an unlockable passive item in The Bimbing of AIsaac: Reedged, a mod for The Binding of Isaac.
Description
Upon taking damage, 50% chance to turn all non-boss enemies in the room into worms/maggots.
30.77% - Round Worm
23.08% - Small Maggot
15.38% - Charger
15.38% - Spitty
7.69% - Conjoined Spitty
3.85% - Night Crawler
3.85% - Roundy
If Isaac has Tarot Cloth, the pool is different:
50.00% - Round Worm
37.50% - Small Maggot
12.50% - Charger
Worm enemies always turn into Small Maggots.
In flooded rooms, any Small Maggots this item spawns are replaced with Small Leeches.
How to unlock?
Defeat Ultra Greedier as 
AIsaac.

Trivia
The meme started when one of the viewers misspelled "a room full of worms" as "a room fool of worms".
After that people started calling AIsaac "such a fool (of worms)".
The meme almost got forgotten until the mod developers started an investigation to find its origin and bring it back.
