Torn Card trinket
Torn Card is an unlockable trinket in The Binding of Isaac, added in the Repentance DLC.
Description
Every 15 shots, Isaac will shoot an Ipecac + My Reflection tear with a very high range value.
The tear will follows Isaac until it hits an enemy or lands, where it will explode, dealing damage to Isaac if nearby.

How to unlock?
Defeat Mother as 
Tainted Lazarus.
