Strange Eye is an unlockable passive item in Fiend Folio: RELOADED, a mod for The Binding of Isaac.

Grants Isaac a kill counter that tracks every enemy he has ever killed throughout the entire save file while holding this item.

Increases flat damage to tears shot from Isaac's left eye based on the total number of tracked kills.

The item will level up after a certain number of kills, and these levels determine the flat damage increase to the left eye:

Each level increase will be notified with a pop-up on the screen that can be closed with the K button, or automatically after 5 seconds.

The name of the item will also change and include its current level name as a prefix each time it is picked up.

Starting from rank "Uncharitable", Isaac gets an increased chance to shoot a critical tear from his left eye, which deals x5 damage. This chance is further increased at rank "Gore-Spattered", and increased again at the final rank, "Grandpa's Own".
