Grants different effect depending on the character's gender.

male ( Isaac, Cain, Judas, Samson, Lazarus, Jacob and Esau, Tainted Garlin, Fiend, Golem, Lewis)
×1.5 damage.
-1 shot speed.
All enemies becomes random champion variant.
If entering a room with Isaac's DPS lower than 3.5 × StageNumber, triggers Berserk! and grants Mom's Knife for the room.

+1.5 tears.
-5 luck.
All shop items become 90%-off.
Upon taking damage from Greed, Super Greed or Ultra Greed, 50% chance to immediately kills them, then spawns 1 Member Card trapdoor and 0-2 Credit Cards.

Unknown Gender( ???, Azazel, Eden, The Lost, Keeper, Apollyon, The Forgotten, and other incompatible mod characters.)
